Pediatric oncology has a strong research culture. Most pediatric oncologists are investigators, involved in clinical care as well as research. As a result, a remarkable proportion of children with cancer enrolls in a trial during treatment. This paper discusses the ethical consequences of the unprecedented integration of research and care in pediatric oncology from the perspective of parents and physicians. An empirical ethical approach, combining (1) a narrative review of (primarily) qualitative studies on parents' and physicians' experiences of the pediatric (...) oncology research practice, and (2) comparison of these experiences with existing theoretical ethical concepts about (pediatric) research. The use of empirical evidence enriches these concepts by taking into account the peculiarities that ethical challenges pose in practice. Analysis of the 22 studies reviewed revealed that the integration of research and care has consequences for the informed consent process, the promotion of the child's best interests, and the role of the physician (doctor vs. scientist). True consent to research is difficult to achieve due to the complexity of research protocols, emotional stress and parents' dependency on their child's physician. Parents' role is to promote their child's best interests, also when they are asked to consider enrolling their child in a trial. Parents are almost never in equipoise on trial participation, which leaves them with the agonizing situation of wanting to do what is best for their child, while being fearful of making the wrong decision. Furthermore, a therapeutic misconception endangers correct assessment of participation, making parents inaccurately attribute therapeutic intent to research procedures. Physicians prefer the perspective of a therapist over a researcher. Consequently they may truly believe that in the research setting they promote the child's best interests, which maintains the existence of a therapeutic misconception between them and parents. Due to the integration of research and care, their different ethical perspectives become intertwined in the daily practice of pediatric oncology. Increasing awareness of what this means for the communication between parents and physicians is essential. Future research should focus on efforts that overcome the problems that the synchronicity of research and care evokes. (shrink)

Background: The 21-item Nurses’ Moral Courage Scale was developed and validated in 2018 in Finland with the purpose of measuring moral courage among nurses. Objectives: The objective of this study was to make a Dutch translation of the Nurses’ Moral Courage Scale to describe the level of nurses’ self-assessed moral courage and associated socio-demographic factors in Flanders, Belgium. Research design: A forward–backward translation method was applied to translate the English Nurses’ Moral Courage Scale to Dutch, and a pilot study was (...) conducted to improve readability and understandability. A non-experimental, descriptive cross-sectional exploratory design was used to conduct a survey. Descriptive analysis was used. Participants: The data were collected from a convenience sample of 559 nurses from two hospitals in Flanders. Ethical considerations: Ethical approval was obtained from the university ethics committee, permission to conduct the study was obtained from the participating hospitals. Participants received a guide letter and gave their informed consent. Findings: The readability and understandability of the Dutch Nurses’ Moral Courage Scale were positively evaluated, and the scale revealed a good level of internal consistency for the total scale and all subscales. Nurses’ mean score of the 21-item Nurses’ Moral Courage Scale was 3.77. The total Nurses’ Moral Courage Scale score was associated with age, experience, professional function, level of education and personal interest. Discussion and Conclusion: The Nurses’ Moral Courage Scale was successfully translated to Dutch. The Flemish nurses perceived themselves as morally courageous, especially when they were in a direct interpersonal relationship with their patients. Acting courageously in ethical dilemmas that involved other actors or organizations appeared to be more challenging. The results strongly suggest the important role of education and ethical leadership in developing and supporting this essential virtue in nursing practice. (shrink)

Continuous sedation at the end of life is a practice that has attracted a great deal of attention. An increasing number of guidelines on the proposed correct performance of the practice have been drafted. All of the guidelines stress the importance of using sedation in proportion to the severity of the patient’s symptoms, thus to reduce the patient’s consciousness no more than is absolutely necessary. As different patients can have different experiences of suffering, the amount of suffering should, ideally, be (...) assessed subjectively; that is, via communication with the patient. Continuously sedated patients are often unable to communicate, however, making subjective methods of pain assessment unusable. For these patients, the degree of consciousness is the sole available measure. It therefore seems important to adequately measure how deeply the patient is sedated, thereby allowing sedation to be increased when it is too light and decreased when it is too heavy. This is in accordance with the idea that reducing consciousness is not an ethically neutral act.Although consciousness measuring techniques are a hot topic in anesthesiology, almost no research exists on the use of such techniques in the context of continuous sedation at the end of life. This article aims to review existing techniques to measure consciousness and to evaluate their applicability, efficiency, and invasiveness for patients who are continuously sedated until death.Techniques commonly used to assess the depth of sedation in continuously sedated patients are basic clinical assessment and sedation scales, as they are often considered reliable and noninvasive. These techniques might not be very reliable, however, since it is known that some patients are nonresponsive and yet aware. Moreover, sedation scales require stimulation of the patient (for example prodding, shaking, or providing painful stimuli), and can thus be considered invasive of one’s bodily integrity or dignity. Other techniques, such as EEG (electroencephalography) derivatives, may score better on reliability and invasiveness. Yet these have so far never been compared to sedation scale scores for patients receiving continuous sedation at the end of life.Therefore, we conclude that, for both clinical and ethical reasons, research into the efficiency and applicability of other techniques, such as derivatives of EEG, are urgently needed. BackgroundThe aim of this study is to compare depression and loneliness among adult siblings of people on the autism spectrum, adult siblings of normotypic individuals, and adults raised alone. In recent years, an increasing interest in the perspective of siblings of children diagnosed with autism has been observed, with studies among this population particularly concerned with the developmental trajectories of children and adolescents at “high risk” for ASD, rarely focusing on their mental well-being.MethodsThe respondents filled out: the survey on sociodemographic (...) data designed by the authors, Beck Depression Inventory II, and De Jong Gierveld Loneliness Scale.ResultsA rise in BDI and an increase in the DJGLS score were predicted by having a sibling diagnosed with ASD. Those effects were independent of subjects’ sex, educational status, place of residence, or a number of siblings.ConclusionThe results underline a fundamental need for the development of mental hygiene programs for families where children with autism spectrum are accompanied by healthy siblings. From surrealism’s beginnings around a Parisian séance table, it oscillated between the occult and the political. One of its key methods, automatism, provided access to both the esoteric and the exoteric: it took form in the mid-19th century as a spiritualist technique for communicating with the other side while, simultanously, this other side could address political issues as equal rights, de-colonisation and a utopian future with an authority coming from beyond the individual. By tracing the development of automatism, the article (...) shows how automatism in surrealism became a call for both a re-orientation of life and an institutional re-organisation by becoming a divination tool for a future community looking back to hermeticism to find a way forward. The article argues that not only can surrealism fruitfully be understood in the light of an occult revival in reaction to crises but, additionally, that it marks the return of and a reaction to a kind of magical thinking in the modern – due to waning religious and socio-economic orthodoxies – that echoes eerily into our own big data contemporary of social medias where we tend to substitute equations with associations. This volume explores how Catholicism began and continues to open its doors to the wider world and to other confessions in embracing ecumenism, thanks to the vision and legacy of the Second Vatican Council. It explores such themes as the twentieth century context preceding the council; parallels between Vatican II and previous councils; its distinctively pastoral character; the legacy of the council in relation to issues such as church-world dynamics, as well as to ethics, social justice, economic activity. The book also contains a wide and original range of ecumenical considerations of the council, including by and in relation to Free Church, Reformed, Orthodox, and Anglican perspectives. One scene from Wim Wenders’ film “Alice in the cities” provokes us to think again about childhood. It’s a fresh polaroid snapshot, where we can see the faces of an adult (Phillip Winter) and a child (Alice) blending and mixing. Starting from this image, this paper makes its way through the movie’s plot, reflecting on the power of freedom that arises from an encounter with children and childhood. First, we examine the relationship between Winter and his job as a photo-journalist, (...) discussing the character of his existential anguish and his supposed indisposition to becoming, which seems to be clear in the way that he reads his own photographs. In a second moment of the film, we understand Alice’s arrival on the scene as something capable of dispersing the circumstances that are constraining him by hurling him into the unpredictable. This event is crystallized in one poetic gesture: the girl confronts the adult with her own face by drawing a self-portrait. Facialization and deterritorialization are twoconcepts that we weave into our discussion, working off the philosophical thinking of Georges Didi-Huberman, Gilles Deleuze and Félix Guattari. Finally, we propose that worry as a value, characteristic of children and travelers, can produce an opening that offers an escape into a freedom of release–or, so to speak, a defacialized freedom. This culture possessed its own institutions, traditions, and rituals that connected its members across borders and religious divides. A professor from Lisbon, a librarian from Hanover, and a schoolmaster from Turku would all speak nearly the same language and wear nearly the same clothing. They would (...) share many of the same dreams, celebrate many of the same heroes, and have similar memories from their youth and adult life. They were convinced that they belonged to the same cultural group and were normally willing to make personal and economic sacrifices when called upon by members of this group. Long before the emergence of European nation-states, the community of scholars formed what Benedict Anderson has called an imagined community, united by the forces of language, culture, and writing.1From around 1500, the community of scholars went by the name respublica litteraria—the Republic of Letters. Italian humanists already used the metaphor in the early fifteenth century, and with Erasmus of Rotterdam's Antibar-barorum liber, it became commonplace within Europe.2 To Erasmus and his [End Page 421] circle the Republic of Letters signified the natural community between all students of creation, divine reason, and Greek and Roman antiquity. It gathered scholars across space and time and set them apart from the hordes of ignorant barbarians. To most early modern scholars, however, respublica litteraria signified much more than a community of reason and knowledge. They used the metaphor to describe every aspect of the culture of scholarship that connected its members and demarcated them from surrounding society. Universities, academies, and secret societies were talked about as parts of the Republic of Letters. Scholarly letters and conversations tête-à-tête, books, lectures and seminars, rituals and traditions were all included. The metaphor demarcated invisible borders between learned and lay, tacitly known to insiders but rarely strictly defined.3The Republic ChallengedDuring the last decades of the seventeenth century the metaphor of the Republic of Letters lost much of its force and elasticity. Scholars used and debated the metaphor more than ever before, but these heated and emotional discussions were not necessarily—as is assumed in much of the work on the eighteenth century4 —a sign that the metaphor and the culture it represented had gained ground. The discussions were, much rather, a symptom of crisis. The end of the great religious wars and the emergence of modern centralized states forced scholars to rethink their role in society and their relationship to the new military and administrative elites. Philosophical skepticism and philological criticism threatened established academic teachings and methods.5 Debates about the Republic of Letters reveal the scholars' efforts to answer these political and intellectual challenges. In her studies of learned Huguenots who, after 1685, were spread across Europe, Anne Goldgar has described how the alienated community of scholars at the turn of the eighteenth century came increasingly under pressure.6 The Huguenots felt marginalized in their adopted [End Page 422] homes, lost income and patronage, and suffered a decline in social prestige. According to Goldgar, these external pressures resulted in stronger internal bonds, as exiled scholars rallied around the ideal of the borderless Republic of Letters. But already before Louis XIV's revocation of the Edict of Nantes, Huguenot scholars defined a French version of the republic—La République des Lettres—in strong opposition to the forces suppressing the Protestant minority. In 1684 Pierre Bayle defiantly declared from his Dutch refuge,It is not a matter of religion, but of science: therefore one should put an end to all boundaries that divide men into different camps, and consider only that which unites them, namely their rank as noblemen in the Republic of Letters. In this sense all scholars should regard themselves as brothers, and consequently one another as family. Kymlicka has offered an influential luck egalitarian justification for a catalogue of polyethnic rights addressing cultural disadvantages of immigrant minorities. In response, Quong argues that while the items on the list are justified, in the light of the fact that the relevant disadvantages of immigrants result from their choice to immigrate, (i) these rights cannot be derived from luck egalitarianism and (ii) that this casts doubt on luck egalitarianism as a theory of cultural justice. As an alternative to Kymlicka’s argument, (...) Quong offers his own justification of polyethnic rights based on a Rawlsian ideal of fair equality of opportunity. I defend luck egalitarianism against Quong’s objection arguing that if choice ever matters, it matters in relation to cultural disadvantages too. Also, the Rawlsian ideal of fair equality of opportunity cannot justify the sort of polyethnic rights that Quong wants it to justify, once we set aside an unwarranted statist focus in Quong’s conception of fair equality of opportunity. Whatever the weaknesses of luck egalitarianism are, the inadequacy of the position in relation to accommodating cultural disadvantages of immigrants is not among them. (shrink)

Une réflexion sur la tradition sous les auspices de Kasper implique une mise en oeuvre de la catholicité qui reconnaît, accepte et promeut la diversité des situations et des cultures. Il s’agit d’une catholicité qui, en tant que système ouvert, conjoint unité et diversité. Mais alors que chez Kasper cette catholicité est pensée seulement dans le contexte oecuménique de l’Église, il semble possible de l’appliquer à l’Église catholique considérée en elle-même. On parvient ainsi à poser une pluralité d’expressions (...) de foi sans briser l’unité. Promouvoir une telle catholicité signifie que l’avenir d’une foi vivante, d’une proposition du message chrétien à la fois crédible et plausible, n’est possible qu’avec des témoins qui soient effectivement et réellement sujets responsables. The relatively new practice of continuous sedation at the end of life (CS) is increasingly being debated in the clinical and ethical literature. This practice received much attention when a U.S. Supreme Court ruling noted that the availability of CS made legalization of physician-assisted suicide (PAS) unnecessary, as CS could alleviate even the most severe suffering. This view has been widely adopted. In this article, we perform an in-depth analysis of four versions of this ?argument of preferable alternative.? Our goal (...) is to determine the extent to which CS can be considered to be an alternative to PAS and to identify the grounds, if any, on which CS may be ethically preferable to PAS. (shrink)

The relationship between the universal and the particular church was the subject of a comprehensive public debate between two German bishop theologians, namely Joseph Ratzinger and Walter Kasper. The debate was initially occasioned by an official document issued by the Congregation for the Doctrine of the Faith on the church as communion . The debate has clearly exposed ecclesiology’s complexities and tensions. Ratzinger taps biblical and theological sources in order to valorise unity or universality in an era of farreaching (...) pluralisation. His argumentation culminates in an endeavour to counter unilateral and horizontal ecclesiologies with an axiomatic ‘ontological and temporal primacy of the universal church over the particular churches’. Kasper recognizes genuine dangers in the one-sided ‘horizontalisation’ and fragmentation of ecclesiology. Rooted in his pastoral and oecumenical experience, he is of the opinion that the theological space created by Vatican II for the particular churches and the diocesan bishops should be claimed to the full. He detects a centralistic deformation of Vatican II in Ratzinger’s axiom. For Ratzinger Kasper’s standpoint can be the first step in an empirical reduction of ecclesiology. In this contribution, I have tried to demonstrate that Kasper’s ecclesiological perspective cannot be suspected of ‘de-theologisation’. Indeed, ‘re-theologisation’ can be understood as the central theme of his theology. The problem of determining the relationship between the universal church and the particular churches is given concrete form in the problem of determining the relationship between the office of bishop, the college of bishops in union with the pope, and the Petrine office. Kasper’s insistence on a legitimate degree of latitude for diocesan bishops does not imply that his goal is to decentralise the Petrine office both theologically and pastorally. At a certain moment in the debate, both theologians recognized that their differing perspectives were more a matter of theological opinion than of essential doctrinal conflict. Their mutual support of the idea of the ‘priority of internal unity’ is a crucial and unmistakable key concept for the correct understanding of the debate. (shrink)
